弱視治療與某些情緒憂傷有關

  177位兒童中有144位(81%)完成父母的問卷,平均年齡是48±5.0個月,及66位中的45位(68%),平均年齡是61±5.8個月,還有151位中的78位(52%),平均年齡是67±5.0個月。
  
  4歲兒童中有77%及5歲兒童中有73%表示配戴眼罩是有困難的,在戴眼鏡中有42%與53%覺得是有困難的;配戴眼罩比戴眼鏡更令兒童不舒服(4歲兒童,P=.03;5歲兒童,P=.01),4歲兒童的父母同樣這樣覺得(P=.01)。

Aug. 9, 2004 — Amblyopia treatment for unilateral visual impairment is associated with some emotional distress but no impact on global well-being or behavior, according to the results of a randomized trial published in the August issue of Ophthalmology.

"Frequent noncompliance with patching implies that treatment is not easy and may cause the child undue emotional stress, outweighing any benefits from an improvement in vision," write S. Hrisos, from the University of Newcastle upon Tyne, in the U.K., and colleagues.

In a prospective, multicenter, randomized controlled trial, 177 children referred from preschool vision screening with a unilateral visual impairment were randomized to receive either glasses with or without patches, glasses alone, or treatment deferred for one year. A self-completion questionnaire, including a psychometric behavioral scale, was sent to the parents of all 177 children recruited to the trial at age four years, to 66 children whose deferred treatment began at age five years, and to 151 children remaining in the trial at the end of the follow-up period. Main outcome measures were the Revised Rutter Parent Scale for Preschool Children, and parental responses to questions concerning the child's general well-being and difficulties related to treatment.

Parental questionnaires were completed for 144 (81%) of 177 children at a mean age of 48 ± 5.0 months, for 45 (68%) of 66 children at a mean age of 61 ± 5.8 months, and for 78 (52%) of 151 children at a mean age of 67 ± 5.0 months.

Eye patching was reported to be difficult by 77% at age four years and 73% at age five years, whereas glasses alone were reported to be difficult by 42% and 53%, respectively. Children were reportedly more upset by patching than by glasses only (P = .03 for age four years and P = .01 for age five years), as were the parents of four-year-olds (P = .01).

Despite these difficulties, most parents thought their children were happy, cooperative, and good tempered, and behavioral scores were similar in both treatment groups.

"Treatment for unilateral visual impairment is not easy to implement and is commonly associated with some degree of distress," the authors write. "Despite this, no impact on the child's global well-being or behavior was seen either during or after the treatment period."
